Documents for recognition of outgoing/incoming mobility
STUDENT OBLIGATIONS DURING MOBILITY AND RECOGNITION PROCEDURE AFTER RETURN FROM Erasmus+ SMP (INTERNSHIP)
During mobility, students must follow the instructions provided by the University of Zagreb, while also fulfilling their obligations to their home institution, the Faculty of Veterinary Medicine, University of Zagreb, for the recognition of completed internship or volunteering, including volunteering as a recently graduated student (Recent Graduate Status), upon return. Every student who completes the clinical part of the internship as part of Erasmus+ SMP mobility must arrange with the Student Affairs Office the dates for attending classes at the Veterinary Directorate and the Croatian Veterinary Institute (CVI). Attendance at classes and obligations towards the Veterinary Directorate and CVI apply only to students in the 12th semester.
The student’s obligations during the mobility period are as follows:
- To regularly attend professional practice within the scope prescribed and defined by the Agreement.
- To keep a Professional Practice Diary of the Faculty of Veterinary Medicine in English.
- To submit Monthly Reports (form available on the University website at https://www.unizg.hr/suradnja/medunarodna-razmjena/razmjena-studenata/strucna-praksa/dokumenti-i-obrasci/smp-202425/)
no later than the 15th of each month to erasmus@vef.unizg.hr,with detailed descriptions of cases in professional-clinical work written in English, at least one and a half pages and a maximum of two pages of text (detailed case descriptions).
- After the mentor at the receiving institution and the student have signed the report, the ERASMUS+/ECTS coordinator will review the submitted document, verify it with a signature, and return it by email if it is acceptable.
Note: The Monthly Reports (in English) must clearly and concisely reflect what was written in the Professional Practice Diary.
Monthly Reports must also be submitted by all students who are volunteering, as well as those undertaking professional practice or holding the status of recent graduate.
Obligations of the student upon return from mobility: Upon returning from mobility, the student at the Faculty of Veterinary Medicine, University of Zagreb, initiates the procedure for recognition of the completed mobility based on the following documents:
Application for recognition of professional practice (form).
- Submit the written request to the Admissions Office of the Faculty.
- Copy of the Learning Agreement for Traineeship in PDF format.
- Copies of Monthly Reports.
- Certificate of completed professional practice (After Mobility Certificate) signed and stamped by the receiving institution.
- Completed and signed Diary of Student Practice, if the student is entitled to ECTS credits, which is submitted to the Office for Integrated Undergraduate and Graduate Studies upon return.
After reviewing the attached documentation, the Vice-Dean for Integrated Studies and Students, on the proposal of the ERASMUS+/ECTS Coordinator, issues a Decision on the Recognition of Professional Practice Mobility.
The Decision on the Recognition of Mobility is delivered by e-mail to the student at the official @vef.hr domain and to the Office for Integrated Undergraduate and Graduate Studies, where the achieved mobility (professional practice) is entered into the ISVU system or the Diploma Supplement.
The student submits the Decision on the Recognition of Professional Practice or Volunteering from ISVU, together with other required documentation, to the Central Office for International Cooperation of the University of Zagreb by e-mail or as specified in the University’s instructions.
